Every week, another team ships an AI agent demo that impresses everyone in the room. And every month, another one of those demos quietly dies before it reaches production. The failure rate isn't because the models are bad. It's because the systems around them don't exist.
The demo trap
A demo is a controlled environment. You pick the inputs, you know the expected outputs, you handle the happy path. Production is the opposite. Production is unexpected inputs, edge cases, concurrent users, rate limits, model outages, cost spikes, and data that doesn't look anything like your test set.
The gap between demo and production isn't a small step — it's a canyon. And most teams don't realise how wide it is until they're already falling.
Five ways agents fail in production
1. No orchestration layer
Most agent demos are single-threaded. One prompt, one response, maybe a tool call. Production workflows are different. They're multi-step, multi-agent, with dependencies between tasks, branching logic, and human approval gates.
Without an orchestration layer, teams end up building bespoke workflow code for every deployment. This code is fragile, hard to test, and impossible to monitor. When it breaks — and it will break — nobody knows where in the pipeline the failure occurred.
2. Uncontrolled costs
In a demo, you don't care that a single request cost £0.50. In production, when that request runs 10,000 times a day, you care a lot. Most agent systems have no cost awareness whatsoever. They send everything to the most capable (and expensive) model, regardless of task complexity.
A well-designed system routes simple classification tasks to cheap models and reserves frontier models for genuinely complex reasoning. This isn't just optimisation — it's the difference between a system that's economically viable and one that bankrupts the project.

When an agent makes a decision in production, someone needs to know what it decided, why, and what data it used. Most agent systems have zero audit trail. No logging of intermediate reasoning. No approval gates for high-stakes decisions. No way to explain to a regulator or a client what happened.
This isn't a nice-to-have. In regulated industries — finance, healthcare, legal — it's a hard requirement. But even in unregulated contexts, you need to debug failures. And you can't debug what you can't see.
4. No error recovery
Demos don't handle errors because demos don't have errors. Production systems fail constantly — API rate limits, model timeouts, invalid tool responses, upstream data quality issues. A production agent system needs retry logic, circuit breakers, fallback models, and graceful degradation.
Most teams discover this the hard way, at 2am, when the agent has been stuck in a retry loop for three hours and has burned through the monthly API budget.
5. No observability
You can't improve what you can't measure. Production agent systems need real-time visibility into:
- Task throughput and latency - Model selection decisions and their rationale - Cost per task, per pipeline, per workspace - Error rates and failure patterns - Quality metrics on outputs
Without this, you're flying blind. You don't know if your system is getting better or worse. You don't know which models are delivering value. You don't know where to optimise.
What production-ready looks like
A production-ready agent system has several properties that demos lack:
Orchestration — A proper execution engine that manages task dependencies, handles branching logic, and supports human-in-the-loop approval gates. Not custom scripts. A real orchestration layer.
Economic routing — Intelligent model selection based on task complexity, cost constraints, and quality requirements. Every task goes to the cheapest model that can handle it well enough.
Governance — Audit trails on every decision. Approval workflows for high-stakes actions. Budget ceilings that actually enforce. Permission systems that limit what agents can do.
Resilience — Retry logic, circuit breakers, fallback models, timeout handling, and graceful degradation. When things go wrong — and they will — the system recovers instead of crashing.
Observability — Real-time dashboards showing cost, throughput, quality, and errors. The ability to trace any output back through the pipeline to understand how it was produced.
The infrastructure problem
The reason most teams don't build these things is simple: it's hard. Building a robust orchestration layer is a months-long engineering effort. Building economic routing requires understanding model capabilities and pricing across providers. Building governance requires thinking about compliance, audit, and access control.
Most teams just want to ship their agent feature. They don't want to build infrastructure. But without the infrastructure, the feature doesn't survive contact with production.
